What to Bring
Please arrive 15 minutes prior to your scheduled evaluation time. This will allow time to review insurance information and to fill out the necessary paperwork. You will need to bring your insurance card, any relative paperwork, along with a copy of the referral form from your doctor, unless the physician sent it to us directly.

What to Expect
Your first appointment will take approximately one hour and will consists of an evaluation with the physical therapist as well as some treatment. Thereafter, your treatment will last anywhere from 1 to 1 1/4 hours depending on your condition and the depth of your program.
